Modifier
A two-character code appended to a CPT/HCPCS code to add detail - such as that a service was distinct, bilateral, or reduced.
Missing or incorrect modifiers are a frequent, avoidable denial reason. The right modifier tells the payer exactly how a service should be interpreted and paid.
